All NEW board members will need to complete the Youth Protection documentation consisting of:

• background screening,
• adult profile,
• media release
• code of conduct form

prior to their Installation on the Advisory Board. They will not need to complete the Adult Profile Update as the first time paperwork will suffice. See PDF forms and information. Connect to the Background Screening site, download the PDF and fill it out online then print out and sign or print it out and fill it out. Please complete all paperwork and return to the Supreme Deputy by mail, or scan and email. (Electronic updates will be needed each year in the future, but the Supreme Deputy needs a hard copy of the completed forms to have on file for first time members.
